The Institute is designed for intermediate to advanced ASQ users who already oversee ASQ implementation and ASQ training within their organization; participants will leave the Institute prepared to instruct colleagues on ASQ-3 and/or ASQ:SE-2. However, new ASQ users also may find the Institute beneficial to their understanding and implementation of ASQ-3 and/or ASQ:SE-2. Meet Your Presenters


Jane Farrell, M.S.,

Jane Farrell, M.S., is an Early Intervention/Early Childhood Special Education Specialist for EC CARES in Lane County, Oregon. Jane received her master’s degree from the University of Oregon Early Intervention Program. She was the original ASQ Outreach Project Coordinator, providing training and consultation on systematic use of the ASQ in 25 states.

 

LaWanda Potter, M.S.

LaWanda Potter, M.S., is an administrator for EC CARES, an early intervention/early childhood special education program in Oregon. She received her master’s degree in child development and family studies from Purdue University. Ms. Potter has been involved with several research studies on ASQ, including questionnaire revisions, data analysis, and documentation. She has also provided outreach training on ASQ across the United States.

What you need to know about registration



Please note that we have a specific number of seats available for the April 2017 Institute. The demand is high and seating is limited and will fill quickly. We cannot guarantee that seating will be available through the registration cutoff date of March 12, 2017. Registration may be closed prior to the cutoff date due to reaching maximum capacity.

How do I get from the Airport to the Hotel if I rent a car?

Rental car companies will provide directions and often a map of the local area; however, the directions below have been provided by the Hotel.

Coming from the McCarran International Airport?

  • Go north on Swenson.
  • Stay in left lane to Harmon Ave.
  • Turn right (north) onto Paradise Road for 1.5 miles.
  • You will find the Embassy Suites Convention Center Las Vegas three blocks past Flamingo road on the right hand side of the road.

Do I need to pay to park at the Hotel?


Complimentary self-parking is available at the hotel.

LODGING

Are hotel accommodations included in the registration fee?


No, for the Institute, you will need to make your own hotel reservations.

A limited number of rooms have been reserved at the Embassy Suites by Hilton Convention Center Las Vegas for a discounted rate of $119.00 per night for single occupancy and $139.00 per night for double occupancy. The room rate includes a complimentary cooked-to-order breakfast as well as a complimentary reception nightly for registered hotel guests only. Rooms are on a first-come, first reserved basis. The hotel may not be able to extend the discounted rate beyond the room block or the cutoff date. To qualify for these rates, please make your reservations no later than March 12, 2017, and specify the code XAQ.

Though there are many lodging options in Las Vegas, we have negotiated a discounted room rate at the Embassy Suites by Hilton Convention Center Las Vegas. When possible, please take advantage of this discounted opportunity and stay in the same hotel as the training.

Are hotel accommodations for single or double occupancy?


All rooms are two room suites which include either a king bed or two double beds and a living room area. The discounted rate of $119 is for single occupancy in a king bedded room and $139 is for double occupancy in a two bedded room. Note that neither rate includes applicable state or local taxes, surcharges, parking, or other incidental fees. An additional fee of $10.00 per person will be added for the third and fourth person staying in a room. The room rate includes a complimentary cooked-to-order breakfast as well as a complimentary reception nightly for registered hotel guests only.

Will I need to pay a deposit on the hotel room or will my credit card be sufficient?


No, the hotel does not require a deposit. However, when you make a reservation, they will ask for a credit card to guarantee the reservation.

What time can I check-in at the Hotel?


Check-in is 4:00 p.m.; if you arrive early and expect to check-in there will be a $25 early check-in fee. If your hotel room is not available, you may check your luggage and enjoy Las Vegas until check-in time.

When do I need to check-out of the Hotel?


Check-out is 11:00 a.m.; unless you are staying for an additional night, you will need to check-out on April 6, 2017 by 11:00 a.m. Luggage storage is available at the hotel. If you need to check-out after you have checked-in but prior to your scheduled departure date, the hotel will charge a $75 early departure fee.

Do I need to bring any ASQ materials to the Institute? / What products are required for the training?

Yes, you will need to bring the following with you:

  • ASQ-3 User’s Guide (Brookes Publishing Stock #70045, $50.00); and
  • ASQ:SE-2 User’s Guide (Brookes Publishing Stock #79581, $50.00).

You may bring the required User’s Guides from your Organization or if you don’t have these materials you may purchase the required materials from Brookes Publishing by either going online to www.brookespublishing.com or by calling us at 1-800-638-3775. Institute attendees receive a 20% pre-Institute discount on all ASQ-3 and ASQ:SE-2 products (except ASQ Online). A discount code will be emailed along with your confirmation of registration within four weeks of registration. At the Institute, we will provide you with a binder of practice forms and handouts. Participants from the same organization may share ASQ-3 User’s Guide and ASQ:SE-2 User’s Guide.

Do I need to bring the ASQ-3 and/or ASQ:SE-2 Questionnaires or Starter Kits to the Institute?


No, you do not need to bring the full Starter Kit or the Questionnaires to the Institute. Please bring only your ASQ-3 User’s Guide (Brookes Publishing Stock #70045, $50.00); and ASQ:SE-2 User’s Guide (Brookes Publishing Stock #79581, $50.00).

If I don’t have the required User’s Guides, how can I purchase them?

What are the hours of the training?


The Institute hours are 8:30 a.m. to 4:30 p.m. on Tuesday, April 4; Wednesday, April 5; and Thursday, April 6, 2017; this schedule is tentative and subject to change.

For the best possible Institute experience and to successfully complete the training, please plan to stay until 4:30 p.m. on Thursday, April 6. Certificates of completion and the flash drive of training materials will be distributed at the conclusion of the training. Please take into account the travel time to and from the airport when making your flight reservations and try your best to plan your travel for after 7:00 p.m. on Thursday, April 6, 2017.

I am new to using ASQ-3 and/or ASQ:SE-2, is this Institute for me?

I already use ASQ-3 and ASQ:SE-2, what can I expect from this training?


The Institute is your opportunity to network with colleagues, have questions answered by our expert speakers, and learn how to conduct an ASQ-3 and/or ASQ:SE-2 introduction seminar within your own organization.

Do I need to attend the full Institute training?


Yes, in order to receive your certificate of completion and flash drive of training materials, you must be present for the appropriate hours each day through the conclusion of the third day. Please take into account travel time to and from the airport when making your flight reservations and try your best to plan your travel for after 7:00 p.m. on Thursday, April 6, 2017.

I need to leave early on the last day to catch my flight home. Is that okay?


It is strongly recommended that departing flights be made for late evening as full attendance is required for successful completion of the training. The seminar ends on Thursday, April 6, 2017, at 4:30 p.m. Please take into account the travel time to and from the airport when making your flight reservations and try your best to plan your travel for after 7:00 p.m. on Thursday, April 6, 2017.

During the training I will need to call my office for a meeting. Is that okay?


We understand that emergencies occur and that a call to the office may become necessary; outside of emergency circumstances, please try to avoid scheduling calls/meetings while you are in attendance at the Institute. If this is unavoidable, please attempt to limit these interruptions by scheduling them during breaks and lunches. As is noted, full attendance is required for successful completion of the training, please arrange your business calls and meetings accordingly. The seminar concludes at 4:30 p.m. on Thursday, April 6, 2017.

After the Institute, what can I do with my new ASQ-3 and ASQ:SE-2 knowledge?


You are being trained to return to your place of employment for the purpose of providing introduction training on ASQ-3 and/or ASQ:SE-2 to colleagues within your office/organization/agency. You are not authorized to train outside of your place of employment nor are you authorized to charge a fee for your training.

What can I do with the handouts and flash drive that I receive at the Institute?


You may duplicate the materials for distribution at trainings for colleagues within your office/organization/agency. If necessary, you may charge a nominal fee only to cover the cost to reproduce training materials.

Who are the Institute trainers?


Institute trainers are people who have been involved in the authorship, development, and/or research of the ASQ-3 and/or the ASQ:SE-2. All have extensive experience in early childhood and early childhood intervention. Many are currently professors at universities in the United States.

Will I meet any of the ASQ-3 or ASQ:SE-2 authors at the Institute?


Yes! Our Institute trainers are expert speakers and contributing authors on either ASQ-3, ASQ:SE-2, or both!

REGISTRATION FEES AND PAYMENT

What is the registration fee?


The registration fee for an individual is USD $995.
Early bird registration $945.00; registrations must be postmarked by February 17, 2017.
Groups of 4 or more and paying with one organization payment, $945.00 per person.

Do you offer group rates?


Yes, you can receive a discount on the registration fee when 4 or more persons from the same program attend and register with one payment (by credit card, check, or purchase order).

How do I pay for the training?


You may pay your registration fee with a credit card, check, or purchase order.

What forms of payment do you accept?

Do you accept purchase orders?


Yes, we will be happy to process your purchase order with your registration form; please submit both when completing the registration process. Due to the high demand for attendance, we are unable to hold any registration forms without payment, no exceptions.

What is NOT included in my registration fee?

Neither your transportation nor your hotel accommodations are included in the registration fee. Each attendee will need to secure his/her own transportation to and from the Institute as well as lodging while at the Institute. Evening meals and entertainment are not included in the registration fee.

CANCELLATION OR SUBSTITUTION

I registered but now I am unable to attend, what are my options?


  1. Send a substitute: If you wish to send a substitute in your place, there will be a $75 administrative fee. Both the administrative fee of $75 and the registration form for the new individual must be completed and sent to our office by email no later than March 21, 2017. After March 21, 2017, we are unable to accept substitutions and the participant may either transfer to another seminar in 2017 or cancel their registration and forfeit their payment.
  2. Transfer to another Institute: We would be glad to accept your registration into another Institute based on availability. There will be a $125.00 administrative fee to transfer to another Institute. Additionally, if the registration fee for the new Institute is higher (for instance, the early bird registration has passed) you will be responsible to pay the difference between the original registration fee paid and the current Institute’s registration fee. The administrative fee of $125 must be paid prior to being approved for the transfer.
  3. Cancel: If you must cancel your attendance completely, all cancellations must be received in writing by mail, fax, or email. Cancellations received prior to March 4, 2017, will receive a 75% refund. Cancellations received after March 4, 2017, are non-refundable.

What is your cancellation policy?


Brookes Publishing Co. reserves the right to cancel the Institute and will inform registrants no later than March 4, 2017. Should the training be cancelled, full refunds for the registration fee only will be issued. Brookes Publishing Co. will not be responsible for airline tickets or hotel reservations that were booked in conjunction with the Institute.

Take a quick shuttle over to the station and then purchase tickets for the monorail to easily travel across The Strip! Where does the Hotel offer shuttle service?


The Hotel can shuttle people to the Fashion Show Mall on The Strip or the Las Vegas Convention Center/Monorail Station. The Hotel does not offer shuttle service to the McCarran International Airport

What times does the shuttle run?


Shuttle service runs daily between 7:30 a.m. and 10:30 p.m. by request. Be sure to contact the Hotel’s front desk to plan your trip.

How can I request shuttle service?


It’s important to know that the Hotel’s shuttle service is available upon request only. Be sure to contact the front desk before you plan to head to the Fashion Show Mall or the Las Vegas Convention Center and LVCC Monorail Station.

Would I be able to obtain training elsewhere if I am unable to attend the April Institute?

Do you offer discounts on purchases after the Institute?


Yes, while at the Institute attendees will receive a 10% discount certificate toward future purchases of any ASQ-3 or ASQ:SE-2 products (excluding ASQ Online) when ordered by September 30, 2017.

Institute attire is casual professional; we recommend dressing in layers as the meeting room temperatures may be cooler or warmer than the temperature outside. You may want to bring a sweater or light jacket to the training.

What meals are included in my registration?


The Institute registration fee includes AM Breaks, Buffet Lunch, and a PM Break daily. The Hotel room rate includes a complimentary cooked-to-order breakfast and also an evening reception with complimentary drinks and light snacks from 5:30 p.m. – 7:30 p.m. at the Terrace Bar on the 3rd Floor. Both events are for registered hotel guests only; however, registrants who are attending the Institute but not staying in the Hotel may purchase the cooked-to-order breakfast for $10.95 per person plus tax.

What are my options for dinner in Las Vegas?


Gather with your new friends before going out on the town! The Hotel offers an evening reception with complimentary drinks and light snacks from 5:30 p.m. – 7:30 p.m. at the Terrace Bar on the 3rd Floor.
