However, new ASQ users also may find the Institute beneficial to their understanding and implementation of ASQ-3 and/or ASQ:SE-2. All participants will receive valuable information to advance their skill level and knowledge of the tools.
Meet Your Presenters
Elizabeth Twombly, M.S., is Senior Researcher and Instructor in the Early Intervention Program at the University of Oregon. For more than 20 years, Ms. Twombly has been involved in ASQ research projects, including renorming for the ASQ:SE-2 and the ASQ-3. She has conducted trainings on ASQ and ASQ:SE nationally and internationally, and has provided technical assistance to states on the development of early identification and referral systems for young children.
Jane Farell, M.S., is an Early Intervention/Early Childhood Special Education Specialist for EC CARES in Lane County, Oregon. Jane received her master’s degree from the University of Oregon Early Intervention Program. She was the original ASQ Outreach Project Coordinator, providing training and consultation on systematic use of the ASQ in 25 states.

What are my transportation options?
Hotel Shuttle – Aloft Chicago O’Hare provides 24-hour, complimentary O’Hare airport shuttle service. The shuttle runs every 30 minutes, starting at 5 AM-midnight.
For O’Hare shuttle service between midnight-5 AM., please call the hotel directly at 847-671-4444, ext. 0.
Taxi Service –
There are Taxi Stands at the lower level curb front (outside of Baggage Claim) at each terminal. Taxi service is available to Chicago and suburban destinations. Please make sure to use the designated Taxi Stands to ensure you have a licensed driver. For your protection, do not accept rides from drivers outside the cab stand or on the departure level (second level) of the terminal roadways. All taxis should have a working meter to calculate the fare based on time and mileage.
Fares are based on traffic conditions, but an average fare from O’Hare to downtown Chicago is $30-40

Shared ride service is available to those who wish to share a ride and pay a flat rate to certain destinations if multiple parties are available for the trip. Shared ride fares from O’Hare include:

You may request a ride with Uber and Lyft by connecting to their rideshare app. Uber Black and Uber Taxi will pick up on the Lower Level.
How do I get from the Airport to the Hotel if I rent a car?
Rental car companies will provide directions and often a map of the local area. For more specific directions to the hotel, contact the hotel concierge or visit http://www.mapquest.com/
Do I need to pay to park at the Hotel?
At the time of printing, the Aloft Chicago O’Hare provides discounted on-site parking for $23/day with in and out privileges for overnight guests and $15 rates for local attendees. Additionally, they offer a premium parking lot option for only $35/day.
Are hotel accommodations included in the registration fee?
No, for the Institute, you will need to make your own hotel reservations.
A limited number of rooms have been reserved at Aloft Chicago O’Hare Hotel for a discounted rate of $159 per night for single & double occupancy plus applicable taxes. Please see below for the specific types of rooms available as well as the discounted nightly room rates.Rooms are on a first-come, first reserved basis. The hotel may not be able to extend the discounted rates beyond the room block or the cutoff date. To qualify for these rates, please make your reservations no later than July 27, 2018, and you must identify yourself with the ASQ-3 & ASQ:SE-2 Training of Trainers Institute. At the time of printing, the Hotel’s room rates are subject to applicable sales tax currently 14%.

Will I need to pay a deposit on the hotel room or will my credit card be sufficient?
Each reservation must be guaranteed with one (1) night’s room and tax, either with a major credit card, check or cash. This deposit is due at the time of reservation and prior to cut-off date. Any reservation not canceled in accordance with reservation policies will be subject to cancellation fee of one night’s room and tax, and will be charged against the individual’s deposit. An early departure fee of $50 will apply if a customer departs prior to the confirmed checkout date.
What time can I check-in at the Hotel?
Check-in is 3:00 p.m.; if you arrive early and your hotel room is not available, you may check your luggage and enjoy the Rosemont area until check-in time.
When do I need to check-out of the Hotel?
Check-out is 12:00 p.m.; unless you are staying for an additional night, you will need to check-out on Wednesday, August 22, 2018 by 11:00 a.m. Luggage storage is available at the hotel.
My organization will pay for my hotel room stay with a credit card, what do I need to do?
All hotels have strict policies for payment of the hotel stay. You may be asked to present a credit card at check-in and may incur charges up to the full amount of your stay unless other payment arrangements have been made. Debit cards used for incidentals may be pre-authorized for an amount up to the full amount of your stay. At all times the hotel policies prevail; please contact the hotel directly. Do I need to bring any ASQ materials to the Institute? / What products are required for the training?
Institute attendees receive a 20% pre-Institute discount on all ASQ-3 and ASQ:SE-2 products (except ASQ Online). The discount code will be emailed along with your confirmation of registration, once your payment is processed. What are the hours of the training?
The Institute hours are 8:30 a.m. to 4:00 p.m. on Monday, August 20; Tuesday, August 21; and Wednesday, August 22, 2018. Schedule is subject to change.
For the best possible Institute experience and to successfully complete the training, please plan to stay until 4:00 p.m. on Wednesday, August 22. Certificates of completion and the flash drive of training materials will be given at the end to those who successfully complete the entire Institute. Please take into account the travel time to and from the airport when making your flight reservations and try your best to plan your travel for after 7:00 p.m. on Wednesday, August 22, 2018.

I am new to using ASQ-3 and/or ASQ:SE-2, is this Institute for me?
The 2018 Institute is designed for intermediate to advanced ASQ users who already oversee ASQ implementation and training within their organization; participants will leave the Institute prepared to instruct their colleagues on ASQ-3 and/or ASQ:SE-2.
I already use ASQ-3 and ASQ:SE-2, what can I expect from this training?
The Institute is your opportunity to network with colleagues, have questions answered by our expert speakers, and learn how to conduct an ASQ-3 and/or ASQ:SE-2 introduction seminar within your own organization.
Do I need to attend the full Institute training?
Yes, in order to receive your certificate of completion and flash drive of training materials, you must be present for all three training days and the appropriate hours and complete the full seminar agenda. Please take into account the travel time to and from the airport when making your flight reservations and try your best to plan your travel for after 7:00 p.m. on Wednesday, August 22, 2018.
I need to leave early on the last day to catch my flight home. Is that okay?
Full attendance is required for successful completion of the training. Please arrange your flight accordingly; the seminar ends on Wednesday, August 22, 2018, at 4:00 p.m. Please take into account the travel time to and from the airport when making your flight reservations and try your best to plan your travel for after 7:00 p.m. on Wednesday, August 22, 2018.
During the training I will need to call my office for a meeting. Is that okay?
We understand that emergencies occur and that a call to the office may become necessary; outside of emergency circumstances, please try to avoid scheduling calls/meetings while you are in attendance at the Institute. If this is unavoidable, please attempt to limit these interruptions by scheduling them during breaks and lunches. As is noted, full attendance is required for successful completion of the training; please arrange your business calls and meetings accordingly. The seminar concludes at 4:00 p.m. on Wednesday, August 22, 2018.
After the Institute, what can I do with my new ASQ-3 and ASQ:SE-2 knowledge?
You are being trained to return to your place of employment for the purpose of providing introduction training on ASQ-3 and/or ASQ:SE-2 to colleagues within your office/organization/agency. You are not authorized to train outside of your place of employment nor are you authorized to charge a fee for your training.
What can I do with the handouts and flash drive that I receive at the Institute?
You may duplicate the materials for distribution at trainings for colleagues within your office/organization/agency. If necessary, you may charge a nominal fee to cover the cost to reproduce training materials only.
Who are the Institute trainers?
Institute trainers are people who have been involved in the authorship, development, and/or research of the ASQ-3 and/or the ASQ:SE-2. All have extensive experience in early childhood and early childhood intervention. Many are currently professors at universities in the United States.
Will I meet any of the ASQ-3 or ASQ:SE-2 authors at the Institute?
Yes! Our Institute trainers are expert speakers and contributing authors on either ASQ-3, ASQ:SE-2, or both!

What is the registration fee?
The 3-day registration fee for an individual is USD $995.00 per person.
Groups of 4 or more and paying with one organization payment, $945.00 per person.
Early bird registration $945.00 per person; registrations must be postmarked by June 11, 2018.

Do you accept purchase orders?
Yes, we will be happy to process your purchase order with your registration form; please submit both when completing the registration process. We will not accept nor hold any registration forms unless the purchase order accompanies the registration.
What is included in my registration fee?
Your registration fee will include a 3-day training where participants will learn how to lead an ASQ-3 and/or ASQ:SE-2 introduction seminar for their own program. Additionally, you will receive practice forms and handouts. Continental breakfast, lunch, and refreshment breaks will be provided for Institute participants each day. Upon successful completion of the Institute, you will receive a certificate of attendance as well as a USB flash drive of training materials on Wednesday, August 22, 2018.
What is NOT included in my registration fee?
Neither your transportation nor your hotel accommodations are included in the registration fee. Each attendee will need to secure his/her own transportation to and from the Institute as well as lodging while at the Institute. Evening meals and entertainment are not included in the registration fee.

CANCELLATION OR SUBSTITUTION
I registered but now I am unable to attend, what are my options?
1) Send a substitute: If you wish to send a substitute in your place, there will be a $75 administrative fee. Both the administrative fee of $75 and the registration form for the new individual must be completed and sent to our office by email no later than June 11, 2018. After June 11, 2018, we are unable to accept substitutions and the participant may either transfer to another seminar in 2018 or cancel their registration and forfeit their payment.
2) Transfer to another Institute: We would be glad to accept your registration into another Institute based on availability. There will be a $300.00 transfer fee to transfer to another Institute. Additionally, if the registration fee for the new Institute is higher (for instance, the early bird registration has passed) you will be responsible to pay the difference between the original registration fee paid and the current Institute’s registration fee. The transfer fee of $300.00 must be paid prior to being approved for the transfer.
3) Cancel: If you must cancel your attendance completely, all cancellations must be received in writing by mail, fax, or email. Cancellations received prior to June 11, 2018, will receive a 75% refund. Cancellations received after June 11, 2018, are non-refundable.
Would I be able to obtain training elsewhere if I am unable to attend the August Institute?
Can I buy product at the Institute?
Yes, we will have products on display available to take home with you or ship home! We offer 10% plus free shipping during the Institute. After the Institute, you will receive a discount code for 10% off any purchase after the Institute.
Do you offer discounts on purchases after the Institute?
Yes, while at the Institute attendees will receive a 10% discount certificate toward future purchases of any ASQ-3 or ASQ:SE-2 products (excluding ASQ Online) when ordered by December 31, 2018.(Free shipping is only for purchases AT the Institute.)
What should I wear to the training?
Institute attire is casual professional; we recommend dressing in layers as the meeting room temperatures may be cooler or warmer than the temperature outside. You may want to bring a sweater or light jacket to the training.
What is the weather like in Chicago?
Chicago’s summers are hot and humid so thankfully air conditioning is always available! Daytime temperatures may reach the low-to-mid 80s but temperatures often come with high humidity causing the temperatures to feel warmer. Lows are in the mid-to-high 60s.
